Insurance Verification and Approval Process

We will contact your insurance company to find out if
weight loss surgery is a covered benefit.

***The time it takes to get insurance approval will vary based on
each person's individual plan details, however, our office will contact you
within 5-7 days to communicate insurance criteria & schedule
your first appointment.

Initial Consult, Financial Consult,
Schedule Appointments

***At your first appointment you will meet Dr. Keith, D.O. to discuss
which procedure you are interested in having. You will also meet with
one of our Patient Coordinators to get all of your appointments scheduled.
Your Personal Patient Coordinator will walk you through
the entire process from start to finish!

"Big 4 Consults"

Psych Evaluation, Dietary Evaluation,
Physical Therapy, OWLO Support Group

***Every patient will be required to complete the above prior to surgery.
This does not include any additional evaluations that may be necessary.
These will be discussed in detail at your first appointment.

Pre-Op Orientation & 10 Day Diet Class

Details of this event will be gone over at your initial consultation.

Pre-Operative Discussion (P.O.D.)
